- A minimum of 4 pictures B&W and/or color.
  Photocopies, slides or laser copies will not be
  accepted.

- Pictures must not be smaller than 4" in height.

- Print, read, fill out and SIGN the terms and
  conditions form and include it with your pictures.

- Bio information including the following:
  Name, Country, State, Eyes, Hair, Chest/Bust,
  Waist, Hips, Weight, Dress/Suit, Shoe,
  Experience, Accomplishments and
  Agency/Representation information.

- Your complete name, address, day/night
  telephone numbers ( will remain confidential)

- If you wish to have your pictures returned, please
  add $5 for shipping and handling.

- Be sure to include the correct amount for the
  type of subscription you have chosen with the
  applicable options. If you are not sure as to the
  total cost, submit a request with which options
  you selected and quote will be emailed back.
  Payment must be submitted payable to Model
  Promote. Only International Bank Drafts or
  Postal Money orders in U.S. funds will be
  accepted. No Cash or Personal Cheques!
